Dear Wise Ones,

Searched the archives, but found nothing really specific...

Had a mom present yestserday with extremely sore nipples, looking exactly
like the slides from the Hoover-Clay collection that were labeled as strep.
Baby, a 3 month old who had been ill the previous week while the family
vacationed at the beach. Baby began with a blistered mouth and what sounded
like vesicles. Mom reports baby is better and eating well at this time. Mom
hx notes an emergency appendectomy and subequent strep infection post-op,
back in June. In the past week mom's sister developed strep and Grandma now
has the same sore mouth as baby did with blisters. Obviously all are at the
very least carriers at this time.

If any of you have worked with a mom like this, these are my questions.

- Is a systemic antibiotic going to heal the nipples quickly, or should she
use a topical?
- How long will this probably take to clear?
- Should the baby be treated in any way?
- Do you suggest an interruption of breastfeeding to decrease liklihood of
reinfection? Mom is a trooper who says it doesn't hurt that much!

Mom's pediatrican diagnosed a probable virus when baby presented with the
blisters in the mouth, and no further treatment was recommended. OB saw mom
yesterday and did start oral antibiotics.

TIA for any assistance,
LuAnn Smith RN, BSN, IBCLC
