Thanks to everyone for their posts on this subject a few weeks ago. Here is
an update:
I saw the mom yesterday. Baby is 5 weeks and on the thin side. Pediatrician
has what seems to me reasonable expectations about how much formula to add
with an SNS. 
Mom is on reglan, it has helped and she is supplying about 2/3 of baby's
food. You should see the massage/compression she does to maximize milk
transfer! It's so great. She also pumps after each feeding. We talked about
increasing pumping or nursing times to 10 from 8 per day, which she thinks
she can manage.  
Now that I see her breasts fuller, with more milk in them, I believe she
does have some degree of hypoplasia. I showed her where her right breast is
not rounded on the inner lower aspect, and she said "Oh, that's the one that
produces less, too". She does not seem to have any other conditions that
would cause                                               low milk supply,
so I think we've solved the mystery. I'm going to send her some info on
Goat's Rue, does anyone have a good online source?
