I agree that a sense of fault is attached to the word 'failed'.
 'Unsuccessful' is more neutral.  Which word to use depends on the
message--connotations as well as denotations--that we mean to convey.

Here are a few other phrases to consider:
    involuntary weaning
    involuntarily scrubbed
    inadvertent weaning
    inadvertently scrapped
    unwilling weaning
    lactation subverted by . . .
    breastfeeding undermined by  . . .
    unintentional weaning
    unintentionally terminated

Recalling the LACTNET discussion of better terms for formula, I remember
the startled expressions of some people when they heard an unexpected
phrase such as 'artificial infant food' and I'm sure it got them
thinking a little bit.

Some of us don't speak English as a native language.  I'd like to hear
what is said in other languages.
