I just came across this site:

Click on:
 Daily Infant Feeding Tracker -  To keep track of how much and how often
your infant is eating during those first few critical weeks.

The column headings are Time, Amount eaten or time nursed, Wet diaper,
Dirty diaper, Mood/Demeanor.

OK - it isn't perfect, but it is a good start for someone who needs to
keep track of these things, and provides an easy way to do it. The forms
come in Acrobat Reader and can be printed out. Use a new page for each 24
hrs - there is a space to write the date.

Under *time nursed* I would write the actual time of day the baby nursed,
not the length of time.
