I have no real answer to your main question, but I suspect that Catherine
Watson Genna is right about a posterior tongue tie. There are parts of
your 
post that remind me of my recent experience with my great-grandson, whose

suck assessment felt almost exactly what you describe, and had a very
obvious tongue tie.

However, to avoid misunderstanding on the part of LN readers, 
the word "hyperinsulinemia" means too much insulin, rather than
<pancreas was not producing insulin> Your description of the baby's
symptoms and treatment sounds to me more like hypoglycemia 
(abnormally low blood sugar).
