Diane told us of the infant that died in Chicago from the use of 
powdered formula. It was actually human milk fortifier that sickened 
and killed this preterm infant. The story can be read at

found that many clinicians working in the NICU have no idea that the 
FDA warned them not to use powdered formula of any kind unless there 
was no other option. Perhaps those of you who work in hospitals could 
print this story and post it where it might help save some other infant 
 from life-long health damage or death. I would also make sure to give 
the story to your Risk Management Department as perhaps they might be 
able to help prevent these tragedies.
