Thanks Lynn for your response on the accuracy of the these tests in hospital. I can see the rationale of having a "captive" group and doing the tests early. I just wanted to make sure that there are not missing false negatives by doing the test too early when in fact a lot of the babies may not have been getting protein feeds for 24 hours. Have you confirmed that there would only be false positives with laboratory specialists? We can then provide the parents of those exclusively breastfed babies who were not transferring well at the start some anticipatory guidance so they do not get overly concerned when they get the scary notification.

Of course this brings up a whole other topic which has been dealt with many times on this forum: why the heck do we (in U.S.) discharge babies so early... before lactogenesis II occurs? But looking on the other side of the coin, the family is getting hardly any sleep or rest in our hospitals. Maybe they are better off at home. I just worry about a lot of little ones that go home on a wing and a prayer ( and a phone number which I hope they know to call). Well I've drifted off the topic and at risk of going into a rant...

Happy Friday,
Mary Westra RN, IBCLC
