My sister (an RD with an advanced degree in clinical nutrition) and I
co-managed a breastfeeding woman who had had bariatric surgery
(intestinal bypass).  Though the specific issues are different (your
client probably does not have absorbtion issues), the basic principle of
making sure that every morsel she puts in her mouth packs a nutritional
punch is the same any time intake is restricted.
Make sure the supplement is not sugared, flavored, and vitamin enriched
cow's milk, that is no more helpful than a glass of milk and a vitamin
pill.
Some of the old LLL tricks like baking whole grain breads with extra
wheat germ and soy flour, adding powdered milk to fluid milk to double
the calcium and protein, etc. might be helpful for her as well.
