I am a big advocate of hand expression.  Perhaps every prenatal class should
include a lactating goat, and the mothers can experiment and learn how to
milk a goat first.  It's the same principle.  It seems impossible until you
'get it right' and then once you know how, it's simple.  Every hospital
could have their own goat herd.
