  I just had a client who had painful letdowns, and engorgement that wasn't
  subsiding. she ended up being diagnosed with a staph infection which she had
  evidently picked up in the hospital when she got cracked nipples the first
  day of nursing. So perhaps over supply and engorgement both put pressure on
