Posting with the mom's permission: 

I have a client who is 3 weeks postpartum, baby (her fourth) is generally latching well and gaining great (although mom reports some feedings are better than others) and mom is not sore at feedings.  She has had a large persistent crack around half of the circumference of her nipple since day 5.  It is not terribly sore but she's annoyed that it's not healing after constant use of bactroban (antibiotic ointment) and Lansinoh for a cpouple of weeks now.  She's wondering if there's something about the baby's suck that's causing constant irritation and an inability for this crack to heal.  I suggested Soothies for better moist wound healing.  Is there anything else you wise ones would suggest?  I should mention that she can't give the breast a rest because she's only nursing on the one side as the other side was producing very little milk from the beginning (same thing happened with previous baby).  Also, she's not reporting any symptoms of thrush.  Also, if APNO is indicated, is there a compounding pharmacy in the NJ area that can provide it?
