I would want to reassess the weight carefully. I am not a nicu nurse or
doctor, but can IV fluids replace enough weight so that baby looks ok weight
gain or loss wise? Most babies that I see who are only transferring this
much milk are down at least 7% on day 3, or maybe even more loss. I have
seen a 12 or 14% loss by day 7.

The other factor I would like to throw in here is if baby has only been
transferring this small amount all along, mom may not have the greatest milk
supply (YET). Which could explain the pumping. Did mom get any medications
before or after birth, like mag sulfate, that might retard her supply?

How are his bowel movements? Yellow yet? How many each day since birth? How
many before he started getting supplements?

I agree that supplementing at the breast would be a good way to keep him
there yet provide extra, if he really needs it.

Also, I don't necessarily see this as a mom with no let-down. If your test
weight is accurate, I would say this is a mom with a poor supply due to lack
of stimulation and milk removal. In other words, if baby has not been
removing milk she is not going to have much milk and may even be starting
the involution process. I see this more likely as a baby who can't transfer
milk properly and this has down regulated mom's supply.

I work with too many moms pumping for premie babies who get started pumping
late, say after day 4 to day 7 and they rarely make a copious milk supply.
