Pamphlets."  Click that link then download the pamphlet entitled, "If
Your Baby Is Not Gaining Weight." The actual title on the downloaded
pamphlet will be "Getting Off to a Good Start."  The pamphlet includes
an easy-to-use feeding and output record, as well as information about
normal weight gain and feeding cues.
