I have to add my .2 cents here.  I've been reading the dialogues regarding
ice versus hot versus cabbage for relief of engorgement.  I've been a big
advocate for the cabbage stuff.  Anyways, I saw a lady yesterday for a
consult with history of breast reduction with removal of the entire nipple in
'88, day 3 postpartum, and engorged.  The nipple tissue appears to me to be
rather flat in appearance.  I wasn't sure whether I'd see milk pores or not
for releasing the status of the engorgement.  But, I'd advised the woman to
get her cabbage leaves when I was on the phone with her.  Then, upon my
consult, I instructed her to apply cold compressses, and she immediately
breathed a sigh of relief!  She said "Oh, this feels so good!"  And we
proceeded to relieve her engorgement and get baby latched onto the breast for
the first time with the use of a SNS and nipple shield.  Of course, we tried
without the SNS and without the shield first, but baby seemed unable to latch
to the "flat" nipple tissue even with the tea cup hold.

case.
