The clumps in the milk are from inflammation in the breast...probably
infection.  Sounds like she started out with a smoldering "subclinical"
infection....but is now more angry.

I would use Dicloxicillin 500mg QID X 10 days. (Or clindamycin if she is
penicillin-allergic)  It would be nice if someone could get a "clean catch"
milk sample for culture first, though.
