Hello Andrea and All;
As part of our preparation for Baby Friendly certification, we developed an
informed consent for infant feeding (actually we adapted ours from Cottage
Grove hospital's).  And we recently added to the exclusive bfdg option a
line that reads "I also request that no pacifiers be used."  The risks and
benefits info on this form is about bfdg VS ABM, not about pacifiers, but if
