I've done Skype consultations. I try to refer people to local help, but
sometimes a mom has just exhausted her local resources.

If the family has a high definition webcam, you can see really well, and
the sound quality is usually really good. You can demonstrate with a
doll and breast model or your own body. You just can't touch or examine,
and that can be hard.

I also use Skype for quick follow-ups after office consultations, so I
can help a mom make the positioning and latch work on her own furniture,
or just refine what we've worked on together. We can do more in 5
minutes on Skype than in a long phone or email exchange.
