Robyn,
I would strongly recommend your patient be evaluated for intraductal cancer.  A client of mine presented very much like this only it was several months into her lactation course.  She also had some pulling in around her areola and the baby rejected that side.  Ultrasound could be done, but a mamogram is a better diagnostic tool for this.  Please do not ignore these symptoms.  Intraductal cancer is very deadly.  I hope this turns out to be a benign-if unusual finding.
