Nipples that are white? I'd check to see if the babe is
clenching her or his jaw during the feed.
If that's what is happening, a couple of things that are helpful
(off the top of my head -- check Susan meintz Maher's Overcoming Sucking
difficulties too):
Perioral massage before the nursing, may help relax the jaw muscles.
Feed 'way before baby is frantic -- cuts down on pressure
Use modified Dancer hand position, pull down on babe's chin until
mom feels release (and relief!)
Use alternating cool and warm washcloths on babe's face prior to feed

Discuss the birth with the mom. Is the baby"tight"? (hypertonic)
What were baby's Apgar scores? (Is there some subtle neurologic damage?)
I'm convinced that we are sometimes the first to see subtle signs of
difficulties. It's worth it to hang in there with the breastfeeding
because it's a five-sense treatment modality for these kiddos!
