I believe that this is happening in Calgary. When the mother infant dyad
is released from the hospital (usually within 24 to 48 hrs), the public
health nurse contacts the family within 24 hrs. If the family is agreeable
a home visit and nursing assessment (incl brf assessment) is done on the
mother infant dyad. If within 2 weeks, the infant is re-admitted to the
hospital an audit of the public health and hospital charts are done. The
public health chart clearly indicates how many hrs post delivery was the
discharge and as well how many hrs post delivery the initial public
health contact (by phone) and home visit. My sense is that this is done in
a positive manner to look for gaps, missed cues ect in order to improve
the system for the family.
I believe that this is a good start towards improving the delivery of care
