Dear Barbara

What do I think?  Personally, I think that Cochrane Reviews are pretty
biased and thus not very useful at all.

They achieve their bias, but hide it well, in the studies they select and
thus exclude, which seem to arrive at a pre-determined outcome.  Thus I'm
afraid I don't trust them.

I know several great people who have been involved in the work of compiling
Cochrane reviews, and I respect their work and their judgment in the normal
way of things, but something happens in a Cochrane review which turns
everything on its head.

And so the pattern is repeated with this one.  More can be found at

  The reviewers manage to come up with no useful conclusion at all.  They
fall back on the premise that more research is needed, especially if
feeding according to the baby's need is inconvenient for the mother, when
we know (don't we?) that there are excellent physiological reasons to do
with breastmilk synthesis and breast storage capacity which absolutely
require the mother to breastfeed the baby when the baby wants.

I agree with your unease, Barbara. How disappointing.  Sounds like a
funding proposal more than anything else.

The conclusion really bothered me. It made it sound like because we have no
studies, we have no guidance. I would have thought then we refer to expert
opinion like the ABM, AAP, and WHO, who recommend nursing on demand.
