Dear Lactnet Friends;

This is not a formal policy. These are thoughts for a care plan for
hospital use of nipple shields.

1) As prolactin receptors need priming in the first 2 weeks after birth,
mothers are advised to hand express or pump 3 or 4 times a day in addition
to breastfeeding. We don't know which mothers will have a negative impact
on supply if the shield is used during this priming process. There is also
no research about colostrum transfer through a shield.

2) Teach mothers to apply the shield correctly; to turn it partially inside
out, place it over the nipple, and press on the lip of the shield to flip
it all the way right side out and pull the nipple into the shaft of the
shield.

3) Shield size based on mother's nipple size, recognizing that there are
cases of oroboobular disproportion.  Hospitals should carry shields larger
than 24mm!!

4) On-going follow up by a lactation professional, including pre/post feed
weights with the shield on, until the shield is no longer needed.

5) Baby needs to breastfeed with with a wide mouth; a lousy latch on a
shield doesn't help anybody.
