But...if a mom goes home with a nipple shield she *still* has to pump
after every feed as milk transfer is not as good with a nipple shield.

yes, i do believe in nipple shields as a last resort.  I've told mom's
they're like crutches.  you don't need them forever to be able to walk.
It's just an assistance tool to get baby going.   I think I prefer the
training wheel metaphor.
