Barbara,

Sounds like a very daunting challenge. I have no words of wisdom about
anything else you mentioned, but I wanted to comment on these:

Since the mom has "latch defying nipples" for starters, nothing wrong in
my estimation with shield (thin, silicone one). With an SNS beneath it,
babe may be encouraged by certainty of flow to keep trying. Starter SNS
with soft "bolus bulb" or fingerfeeder with tube under shield would allow
mom to squirt in a bolus into the underside of the shield to "prime" it
and encourage bursts of suckling.

It seems to me the main mechanical stimulus for milk production is going
to have to be the pump for the forseeable future, and the delivery of
calories to the baby, preferably in en face position at the breast, is
going to have to take first priority.

Is it totally unrealistic for the mom to try to pump q. 2 h. during her
waking hours and 2x at night for a total of 10 vigorous stimulations in a
24 hour day, at least for a week or two? Does this mom have any daily
adult help at all? Maybe someone could assist her by holding a single
flange to the other breast several of the times she is nursing.

Growth of the babys jaw and buildup of mom's supply may favor a switch to
bare nipple weeks or more down the pike, but it doesn';t sound like you
can compare what the baby is doing with the shield to the stimulation of
a double hospital grade pump for 10-15 minutes.

After last week's horrible tale of the death of the 12 year old adopted
girl being "rebirthed" in a twisted sheet stuffed with pillows to
simulate a uterus, I am rethinking the use of that term, because it
obviously means different things to different people. In it's place, I
like the phrase someone has used:"remedial co-bathing".

What was this mom referring to? IME, the sensation of water moving over
the surface of the baby's skin is the facet that is stimulating the
largest sense organ in the body in a way likely to elicit prenatal body
memories. I don't think this could be duplicated in bed. I doubt that
even the use of wet towels held to the back of the baby who was skin to
skin with the mother would give the same sensation.

I congratulate you for taking on this challenge and helping this mother
gratis. Stars for your crown (and hers) someday, but right now a big
psychological and physiological leap of faith. Please keep us posted.
