Hi Chris. I believe there is a sample nipple shield consent form in
Riordan/Auerbach's text. I have one that I developed from this one, I believe.
I actually mentioned this today in a lecture to some hospital nurses, and
they were amazed that one would need that. It is not an intervention to use
lightly, or to send a mom off with it never to followup, etc. That's why I
tell folks that they are NOT a substitute for skilled latch help, or as a
stopgap when the night is too busy and one doesn't have the time to properly
help. Nurses are overbooked, and need more time to allocate to breastfeeding.
