<<I know of no better way to poll a wide range of practices than to ask all of
you about your hypoglycemia protocols. Yesterday I encountered a doctor that
wants his newborns to have blood sugars 50 mg/dl and above immediately after
birth. We finally got a protocol that was "livable" involving not doing
blood sugars (or Chem-Strips) on as many babies and then guidelines that said
40 mg/dl and above. >>
We recently changed our protocol from 45 to 40mg/dl. We still do Accucheks on
all our infants. If the 1st is 40 or greater, we do no more unless
symptomatic. If the 1st is less than 40 or the baby is at risk (SGA, LGA,
IDM, etc), we have to get 3 consecutive readings (before feeds) greater than
40 before it is considered stabilized. If the 1st Accuchek reading is less
than 40, we will also send a central blood glucose to the lab for
confirmation.
