It can be very difficult for an oral antibiotic to completely clear a
skin infection without topical assistance. In this case, it may have
helped enough to give some relief, but when the drug was removed, the
remaining bacteria simply re-grew, perhaps more resistant than before.
I would consider culturing the nipple, a new round of antibiotics, and
a topical anti-inflammatory with antibiotic. Being "into" drugs, more
than many, I would also probably add a prophylactic dose of Diflucan to
ward off yeast.
