In my non-scientific study of one woman who had severe PIH and HELLP with
baby #1 (me), the main reason lactogenesis was at all delayed was the
practices of the hospital where I delivered my small but healthy preterm
baby.  Scrub the newborn shiny while she screams herself silly (and then
wants to shut down for hours), put a gigantic vanilla-scented pacifier in her
sweet little mouth, don't bring her to her mother when she is crying but give
her bottles of glucose in the nursery, and tell her mother "honey I formula
fed all my kids and they are just fine" while you hand her a case of ready to
feed just in case.  And then make certain you get a pediatrician who wouldn't
know a good latch from a doorknob.
  It is a wonder we ever got breastfeeding!
