I can't imagine that a therapeutic abortion would have much impact,
if any on milk supply or quality. At 6 weeks, the procedure is basically a D
& E. If she chooses anesthesia, it will likely be a fast-acting barbiturate
or something similar, given IM or IV, which will be out of her system
quickly. Some folks give methergine or antibiotic afterwards, which are both
compatible with breastfeeding.
Remember that LAM is 98% protective against pregnancy up to 6 months
postpartum, which means that 2% of women are not protected completely and
need to use another method while using LAM. Some women are more fertile than
others. Fertility goes up for a time after an abortion, either therapeutic or
spontaneous, so she'll need contraception. She most likely will be having all
sorts of feelings for a while now, having to make this umplanned choice.
Extended follow-up for emotional support would be indicated. Warmly, Nikki
