Lidocaine with epi makes the numbing last longer and also cuts down on bleeding.  You usually want to use the epi for those reasons.  You should not use epi in areas of the body were the blood supply is limited "coming from only one direction"

The way to remember where not to use epinephrine is the short little saying
fingers,
toes,
penis,
nose.
