 I'm hopelessly behind on responding to posts.  Hope this helps for the
future.

When you teach a mother to point the nipple toward the roof of the mouth
as she latches the baby on in order to get plenty of the areolar tissue
into the mouth, this can sometimes lead to a flexible nipple being
folded up so that the tip rubs the baby's palate and results in blisters
for mom.  It only takes a few minutes.  For these moms in particular,
you need to be assure that the nipple is pointed toward the back of the
mouth as the baby goes onto the breast.
