Part of the solution to this problem, IHO, is as follows:
1.      The professional nurse assigned to the family for that shift is and
should be the coordinator of all care.   Mothers and fathers need to
be instructed never to hand over their baby to any "hospital employee"
without first checking with either the mother's nurse or the baby's
nurse.  The nurse has the closest contact with the family and so can
advocate for, and protect the family.
2.      Family Centered Care!!!  The empowered parent is less likely to
hand over their baby to just anybody.  At every opportunity caregivers
can encourage parents to be their baby's voice.  If something doesn't
sound right or feel right, encourage them to question.  We need to be
asking the parents permission to hold their baby, perform procedures
on their baby, etc.  When I worked at a local military NICU, we had a
sign made which said  "Dear Parents, you are the most important member
of your baby's care team.  Please let us know if there is anything we
can do for you."
