I have a consent/release for photos as the last paragraph on my general
consent-to-work-with-you form. The client can cross out that paragraph she
does NOT want photos taken. I don't always take pictures, but it puts the
photo-taking in the category of "routine care" so it's not a surprise if I
want to take a picture.
