I do not trust titles on the Internet because I have no way to verify them.  This site could have people impersonating MDs and CNMs for instance.

Just as I do not shoot the messenger, I also avoid trusting the messenger.  I research by consulting local professionals.

I think the use or avoidance of titles is a personal choice, rationally irrelevant to the actual quality of LACTNET communication.
