A Counsellor here in Aus is having a problem with a young mum and her
baby. She has given permission to post this to lactnet for some more
suggestions. Another LC and I have had some input, but progress seems
to be very slow. Maybe it is a problem only 'tincture of time' can
cure - this young lady is determined to breastfeed but getting very
frustrated and disillusioned. She has seen a local LC, rested her
nipples and expressed to help them heal, They don't seem to think
thrush or allergies are a problem. As far as I know, oral anatomy has
been checked - any new ideas?? The message follows:

>Joy and Querida,
>
>Thank you both for your input into Melissa and Tiana's feeding.
Melissa
>has had Tiana back on the breast a few times this week. It is still
>pretty much the same except that Melissa isn't frightened to put her
on
>anymore.
>
>The feed starts off fine. Melissa holds Tiana in a cross cradle hold
to
>get her attached and touches Tiana's bottom lip with the bottom edge
of
>her areola and when Tiana opens her mouth she pushes and flips in as
>much of the areola as possible (as in the Sue Cox video). Melissa
gets
>about 1 1/2 to 2 inches of areola into Tiana's mouth. It doesn't hurt
at
>first. When Melissa gets a let down, sometimes before, Tiana's starts
to
>click and there sounds like a loss of suction as she clicks. This
>eventually hurts. The areola from the outside looks the same. When it
>hurts Melissa takes her off, the nipple is creased from the top of
the
>nipple to the bottom. Melissa's let down is fast.
>
>One thing that she does differently to other people is that Melissa
lays
>Tiana on her bottom arm and holds her top arm behind Tiana's back.
Tiana
>doesn't seem uncomfortable. This is so her arms stay out of the way.
She
>is quite a little Hudini.
>
>I have search the Lactnet archives but haven't really come across
>anything the same. We are going through them though. Melissa is going
to
>try a rebirth and is using Daktarin Oral Gel in both Tiana's mouth
and
>on her nipples just in case it is thrush. Tiana's tongue is slightly
>white.
>
>Anymore ideas. Maybe you could put this on Lactnet for me.
