What I teach moms to do is make a "wonton" (or "taco") with their baby: Fold
a receiving blanket into a triangle. Have her lay on side she plans to nurse
on next. Place a corner of the blanket underneath her, so the fold (long
side) of the triangle is toward her head and about even with her breast.
Place baby on blanket, then bring outside point of triangle over baby & tuck
in on top of first point. Sort of like folding a wonton wrapper or tortilla
shell! This anchors baby snug against mom, & she doesn't have to keep her arm
tight around baby. If she dozes off, baby won't roll away. If mom starts to
roll toward baby, blanket tightens, causing baby to squirm & wake mom up.
Even if mom rolls onto her back, enough of blanket is anchored under her to
keep baby close.

Does this make sense? I also used this yesterday w/a baby who was vacuum-
delivered & seemed to have a headache & shoulder ache, wouldn't settle to
nurse when her head or shoulders were touched. Blanket kept baby positioned
at breast without having any pressure on her head/neck/shoulder area.
