The first thing I thought of was reflux, altho you don't describe those
symptoms and the fact that lying down is preferred by child doesn't fit
either.
Could the baby have herpes lesions in throat or something like? An
injury that isn't apparent? Abuse?
Just throwing out ideas - hope something you get rings a bell. The poor
child sounds so sick.
