Hi, Karen,

Has she tried more frequent nursing?  Broken cabbage leaves on the
affected breast?  How about warm soaks and manual expression before
nursing, followed by cold afterward to relieve the pain?

If the breast is hard, has the baby hurt her nipple trying to latch on?  Is
she tense when she offers the engorged breast?  If so, I'd try relaxation
exercises to try to help her letdown.
