I need help here...history:
Baby-12 days old...born via vacuum suction after getting stuck in birth
canal. Mom was cut/torn all the way through to her rectum. Went into
surgery the next morning to repair her perineum. BFing was not initalized
until baby was over 24 hours old. Mom devolped sore nipples right away.
Consultant came in to see her, tried to get baby to latch...to no
avail...told mom she would be back later. Well, she was not able to get
back to mom. Mom went home later that night w/o baby ever latching on.
Mom contacted me on day 3, c/o sore nipples and severe engorgement. I saw
her in office on Thursday. Nipples were a mess. Huge scabbed all over the
entire nipple area. Baby had a tight frenulum, and a bunched up tounge.
Mom could not even tolorate baby latching on. LC got mom an electric pump,
and had her pump, to get her nipples healed. Talked to mom daily, had a
copious supply..like 4-6 oz. out of each side on day 5 and beyond. Mom
was/is scheduled to get an SNS from our LC, tomorrow, Tuesday. Today mom
calls me and tells me that her supply has dropped, dramtically. Down from
4-6 oz on a side, to 1-2 oz. total out of both sides. Mom says she hasn't
changed anything or started an birth control. Denies any possibility of
being pregnant. Baby is now 12 days old. Both LC and I are at a loss as
to why mom's milk would suddenly go away. Mom told me later in the
conversation that she has been smoking a pack and a half of cigerrets (sp)
since day 4 or 5. Can anyone help me think of any reason why she would
lose her milk supply all of the sudden? She really seems to want to BF,
and dad is very supportive. Why mould mom's supply drop so suddenly and so
dramatically? Mom really wants baby back to breast, and wants to BF for at
least a year, as she is a SAHM. Any thoughts would be appreciated. Also,
please CC: me if you would with any responses. Thank you,
Lisa
