I have found that some nipple types are more difficult to express colostrum
from than others..especially very bulbous with very narrow bases.  When we get
a baby to latch on well, I like to point out the cues that show baby is doing
a better job than our fingers.  Sometimes a breast that is firm (before
filling stage) is more difficult to express manually as well.
