Dear Jay,
This was a case I had about 4 years ago, and her medical record is no
longer at my location, so this account is from my
recollections.......baby had a lovely vaginal birth at our out of
hospital birth center, gained wt. well for the first 2 weeks on
exclusive breastfeeding. (After 2weeks, if they are doing well, we don't
see them again til 6 weeks) Baby came in w/ mom for a 6 week check-up
with CNM, and was in failure to thrive. I referred baby for medical
eval, and the ped had him hospitalized right away. A few days later,
mmom began noting frank blood in baby's diapers occasionally. Baby was
transferred to regional research hospital @ mom's request. Pediatric
gastroenterologist diagnosed allergy to proteins in mom's milk, probably
cow's milk, but told mom there wasn't any point in her eliminating dairy
from her diet and maintaining her milk supply by pumping. By this time
the parents were frightened and demoralized, and mom gave up hope of
breastfeeding. Baby was fed expensive pre-digested formula via NG tube,
and began finally, to thrive. Interestingly for you, the grandmother
recalled that a similar scenario had unfolded when the mother was born,
that she had been started on regular ABM, but had not thrived, and had
"almost died". That's the gist of it, and I hope it helps you out.
Since then I have realized that the blood in baby's stools was from
lesions in his bowel, a result of severe allergy. There was a recent
post on LACTNET (i think fron Robert Cordes) giving references for
bloody stools........Good luck!
