You most likely have thought of this already, but just in case...  Has she
tried getting the baby to nurse wth his chin closest to the lump?  With the
difficult latch maybe this would be too hard, but it's worked really well
with the moms I counsel, some of them have had to be incredibly creative.
And about the dirt grains in her milk, maybe it's something like the rust
colored milk in recent discussion here, but in little bits because of the
plug.  I once had a mom with plugged ducts that when finally releasing came
through her breast looking like cooked spaghetti, several strands 3-4 cm in
length.  Could it be a plug broke up into minute bits and is rusty colored?

To probably the most northern US mom, from one of the more southernly
located (central FL) where it's finally feeling chilly at 65F right now.
