Pamela,
I agreed with most of your suggestions, but don't quite understand this
one--
"at the first hint of engorgement/pain that will not
resolve with massage and drainage seek medical advice about need for
antibiotic to treat mastitis (I realize this may be controversial!) - if
there are areas of the breast that do not drain because of severed ducts
there will be pain/lumpy areas and potential mastitis lasting several
days."
Would you recommend antibiotics for very engorged bottle-feeding moms?
Hmmm . . . as I typed that I remembered that you've reported that almost
everybody nurses in your neck of the woods. I think Maureen Minchin has
written eloquently about the over-use of antibiotics for mastitis (which
just means inflammation, not infection). There was a recent article by
Chloe Fisher and somebody else that talked about this issue (it's on my
desk at the hospital unfortunately). Now, I know that if ducts are severed
you're not going to be able to treat mastitis with improved drainage, but
I'll still bet that antibiotics would not be necessary. We see *many*
bottle-feeding moms in the states that aren't treated with anything, and
they don't need antibiotics. An OTC anti-inflammatory would probably work
just as well.
