Deena: There was a case report in the ABM News and Views, Vol.3, No.1,
1997, by Anne Montgomery, MD. She reported the use of lithium in a
patient who had been well controlled on lithium for years, had stopped it
for her first pregnancy and the year she nursed her first, but with her
second developed severe depression in the second trimester, and took
lithium during the rest of the pregnancy. The lithium was d/c'd at birth,
and breastfeeding initiated well, the mother again needed the lithium.
She restarted it, and the mom and baby were monitored. The baby did well,
developed well, was noted to be fussy. Baby's serum lithium level at 2
weeks was 0.31 (1/2 his mother's level) and TSH was normal. At 1 month,
baby's level was 0.29. At 4 months, baby's lithium was <0.2. Hope this
helps. Alicia.
