  Apendix 1 in Breastfeeding Source Book by Marilyn Grams M.D.  has a
sample letter to an insurance company to expedite coverage of rental of
breast pumps. It works too. My insurance company was not going to pay
pump rental for my premie but they reconsidrered when we sent this
letter. I will send it to anyone who wants me to.
