Children of 3 never eat exactly the same amount each time and trying
to force them to just drives everyone crazy. It is true whether they
are breastfeeding or eating cereal in the morning.
When I was a resident (just about when insulin was discovered) we
tended to keep the insulin amounts on the low side for young children,
because we did not want them to get hypoglycemic. And we tolerated
higher sugars than we would have with older children, because of that.
So the doctors just won't know how much he got.
I would keep nursing in the morning.
