Hello Netters,
Since it was mentioned about Medela's sample insurance letter for pump rental
insurance reimbursement, that can be found in 1987 Rental Roundup newsletter,
Volume 4, # 1.  I also have my own that I would be more than happy to share.
By the way, if you are submitting the insurance letter, Drs. prescription,
 and or the UCLA's superbill, make sure that the codes and diagnosis are all
the same.  There is a better chance of reimbursement if all is placed under
baby's claim.  Have a good weekend all.
