This sounds like a classic case of deep infection with Candida.  Too bad
the docs do not want to hear.  Perhaps if you ask them to contact me, I
will try to discuss this with them.  Email is fine.

Fluconazole 100 mg bid for two weeks would do the trick.  Also
ketoconazole 200 mg bid for two weeks.  If however, you wish to avoid
the doctors, try gentian violet 1% applied in the baby's mouth and on
the nipples once a day for three days.  Occasionally this helps even the
deep pain, don't ask me why.
