Dr. Newman/and LACTNETTERS.
I was called by a mom this morning who is 6 weeks pregnant. Her history
includes:
1 spAb, a ectopic, 5 years of infertility and a 1 year old whom she conceived
with injection assistance. Her progesterone level is 12 and the infertility
specialist says it should be above 20. While he acknowledges he has never
worked with a mom who is nursing, he says that there is some theoretical risk
of miscarrying because
prolactin levels may be associated with low progesterone. She will be
transferring care to a pvt. OB in a week or two. This ob is ok with her
continuing to nurse. She has had occasional spotting, but she thinks it is
because of the progesterone injections she is on. With her daughter she had
spotting in the first and early second trimester.
Her daughter (and family) had a strong history of allergies and thus she was
encouraged to hold off on solids as long as possible. This childs primary
nutrition is from the breast. She is not tolerating any soy, goat or cow's
milk derivatives.
She has gotten the nursing down from 8/day to 5-6/day.
At one year out I would think prolactin levels have normalized, and thus
reduced any theoretical risk or ab. She notices that since conception the
child has shorter duration of nursing episodes, which i thought could be
related to a different composition of the milk now or could just be a natural
development.
Do you have any experience of data that would speak to this?
She sees her infertility specialist for the last time on 4-14. I really
think he would be receptive to your input.
