Just for the record-while I understand the serious concern that a baby who
isn't having a BM and is very young... My second daughter was 3 days on
discharge from the hospital, and didn't have another BM til day 9 when she
was christened-had enough at that time to soak through all her clothing
lavers, and all of mine, to the skin! She follwed with a pattern of saving
stools -at around 3 mos she went twice a week, once waiting 9 days!! And she
was flying off the top percentile of the weight charts(for what that's worth)
So there was a baby who was gaining well but not having a BM each feeding.
They're all different, that's for sure!

Shigella is present in intestinal tract, I don't think it would be in her
milk at any point--maybe they want her stool culture to be negative,  but she
won't give the infection to the baby by breastfeeding--it would be by the
bacteria being present on something going into the baby's mouth-poor
handwashing, for example, in a restaurant or cafeteria setting on the part of
the food preparers.
Is this one of those sacrifices?
